We would like to ask if there are people within your reach that could help us, NHS and social care providers to better understand what people who still need the support of the NHS or social care services want and need when they are discharged from hospital?
There are support services known as reablement and intermediate care services and over the past few months we have been speaking to people using these services to understand their experiences.
But this has highlighted the lack of diversity of people using the service, we heard mostly from patients within white British communities. It has become clear many other sections of our community are not accessing this post-discharge service.
At the moment we are working to understand the reasons why fewer people from ethnically diverse backgrounds access reablement and intermediate care and we’d like to explore why – whether they are offered it initially, whether it’s not suitable and why.
We are speaking to diverse communities face to face and would appreciate it if you can share this with your networks to help us reach people who may not have heard about this:
